21Safety, and what it cost to learn it

The short version: Most of the rules in this guide were written after someone died. The recurring lessons are about crowd density and site geometry, structures rated for the weather, and - over and over - having someone with clear authority to stop the show in time. The UK keeps that knowledge in the Purple Guide; the US in Event Safety Alliance guidance and ANSI standards.

Crowds

Love Parade, Duisburg, 24 July 2010. Twenty-one people died and 652 were injured. A 240-meter tunnel and underpasses fed a single ramp that was effectively the only way in and out; police turned arrivals back by loudspeaker while people kept pushing in from behind. Autopsies found all deaths came from crushed rib cages, and a 2012 scientific analysis described "crowd turbulence", not a stampede451. The trial ended in May 2020 without a verdict, and the Love Parade was permanently cancelled the day after the disaster451. The vocabulary matters: "crowd crush" and "crowd turbulence" are accurate; "stampede", which the analysis rejects, wrongly implies the people caught in it caused it.

Astroworld, Houston, 5 November 2021. Ten people died of compression asphyxia, the youngest aged 9; about 300 were treated on site446. A mass casualty event was declared at 9:38 p.m., and the show ended around 10:10 or 10:15452. Plaintiffs' experts allege the site was planned at 5 square feet per person instead of the fire code's 7, with no density monitoring in the area where all ten were fatally injured, and that the safety lead had written beforehand: "there is no way we are going to fit 50k in front of that stage" - these are allegations, not court findings453. A grand jury declined to indict the headliner, Travis Scott, and five others446.

Structures and weather

Indiana State Fair, 13 August 2011. Seven dead. Sources differ on injuries: some give more than 40, while the Event Safety Alliance's own history and Live Design in 2026 give 58362454. Beyond the engineering failure, one review found preparedness "inadequate" and the emergency plan "not fully developed", and another cited "an ambiguity of authority" over postponing the show455. It was the third weather-related outdoor roof collapse in North America in about three weeks362.

Pukkelpop, Belgium, 18 August 2011. A sudden storm brought down a stage, a large tent and screens with up to 60,000-65,000 people on site, and the festival was cancelled. Early reports said three dead, then five; injury counts ranged from 40 to more than 70456457. Five is the commonly cited final toll, but we could not verify it from an official source.

Who keeps the knowledge

The Event Safety Alliance formed in response to Indiana, led by Linkin Park's tour manager Jim Digby, and debuted at the Indiana Statehouse in April 2012, modeling itself on the UK's Purple Guide362. Its founding date is ambiguous: its origins page says the group formed in early 2012, while it marked its 15th anniversary in 2026, which counts from 2011362454. Both are defensible - the cause was 2011, the organization was 2012. Its Event Safety Guide first appeared in February 2013 and now runs to 39 chapters; as of August 2026 it was developing a second version and an OSHA 10 course for live events362449454. ANSI ES1.9-2020, approved in July 2020, covers crowd management458.

The Purple Guide, originally published by the UK's Health and Safety Executive and taken over by the Events Industry Forum in 2012, is subscription-based, non-prescriptive and runs to 45 chapters, from crowd management and medical to weather, temporary structures and special effects459. The ESA traces it to the aftermath of Hillsborough in 1989362.

Lasers, effects and fire

Covered in their own sections above. The short reminder: no US laser show above 5 mW without an FDA variance, no audience scanning without a specific one, and outdoor beams need an FAA review filed about 90 days ahead223. Indoor sparks near combustible finishes are the pattern behind the deadliest club fires345346348.

Harm reduction

In November 2019 a New South Wales coroner investigating six MDMA-related festival deaths of people aged 18 to 23 made 28 recommendations, including a drug-checking trial "as soon as possible" and an end to drug dogs; the state government opposed pill testing460. In the UK, a new Home Office licensing requirement stopped on-site drug checking in 2023, until the drug-checking charity The Loop received the first license in May 2024461. In the US, DanceSafe (founded 1998) was integrated on site at TomorrowWorld, which the Drug Policy Alliance called a first for a major promoter462 - against the background of the liability fears described under the RAVE Act above.

The common thread

Our reading of the record: across Indiana and Astroworld, nobody with clear authority stopped the event in time. Indiana's reviews say so explicitly; in Houston, roughly half an hour passed between the mass-casualty declaration and the end of the show455452. Every production document in this guide can be perfect, and it still comes down to someone being empowered, and willing, to call it.
